- Let your patients know to download and complete their forms (PDF format). If possible, they should do this before their first visit, to speed up their check-in process. Should they be unable to do so, they can complete the forms in our office.
- Once completed, we’ll automatically receive any information entered by you or your patient. Please note that we check daily, and we’ll quickly get in touch with your patient.
